The top-entry should be preselected. When there's two entries, in 50% of the cases, I'd want to take the top-one. So if that was preselected, I'd be saving time.
If I wanted the bottom one, it would not matter, since I could press up-arrow and select it, instead of pressing down-arrow, if the top entry was preselected.
However if there were three attachments, then I'd be wasting a keystroke no matter what, if I wanted to open the second attachment.
Life's short -> UIs should be fast.
